Understanding Pain: The Key to Lasting Relief

Joint and Soft Tissue

Pain is a universal experience, yet it manifests in different ways and has multiple root causes. Understanding the source of pain is the first step toward effective treatment and long-term relief. In functional physical medicine and rehabilitation, we categorize pain into four primary types: bone and joint (scleratogenous) pain, muscle and tendon (myofascial) pain, nerve (neuropathic) pain, and internal organ (viscerosomatic) pain. Each type has unique characteristics and requires targeted interventions.

Patient Self-Assessment Quiz: Identify Your Pain Source

Answer the following questions to help determine the possible source of your pain:

  1. How would you describe your pain?

    • A. Deep, dull, and achy (Bone & Joint Pain)

    • B. Burning, tight, or knot-like (Muscle & Tendon Pain)

    • C. Sharp, tingling, or shooting (Nerve Pain)

    • D. Deep, diffuse, and sometimes referred to another area (Internal Organ Pain)

  2. Where is your pain located?

    • A. Joints, spine, or bones

    • B. Muscles, tendons, or soft tissues

    • C. Along a nerve path, radiating down an arm or leg

    • D. Abdomen, chest, or referred pain (e.g., shoulder or back)

  3. What activities make your pain worse?

    • A. Weight-bearing, movement, or pressure on a joint

    • B. Repetitive motions, stretching, or muscle use

    • C. Sitting, standing, or specific nerve compression positions

    • D. Eating, stress, or specific body positions

  4. Does your pain change throughout the day?

    • A. Worsens with movement, better with rest

    • B. Worse after activity but relieved with massage or stretching

    • C. Constant or worsens with prolonged sitting/standing

    • D. Comes and goes, triggered by digestion, stress, or other internal factors

  5. Do you experience any additional symptoms?

    • A. Stiffness, swelling, or joint instability

    • B. Muscle tightness, spasms, or limited mobility

    • C. Numbness, weakness, or electric-like pain

    • D. Digestive issues, shortness of breath, or unexplained fatigue

Quiz Results:

  • Mostly A’s: Your pain may be Bone & Joint (Scleratogenous) Pain.

  • Mostly B’s: Your pain may be Muscle & Tendon (Myofascial) Pain.

  • Mostly C’s: Your pain may be Nerve (Neuropathic) Pain.

  • Mostly D’s: Your pain may be Internal Organ (Viscerosomatic) Pain.

Bone and Joint Pain (Scleratogenous Pain)

Scleratogenous pain originates from bones, joints, and the surrounding connective tissue. It is typically deep, dull, and achy, often difficult to localize precisely. This type of pain is common in conditions such as arthritis, spinal misalignments, disc degeneration, and joint instability. Patients often describe it as an aching discomfort that worsens with prolonged activity or pressure.

Common Causes:

  • Osteoarthritis

  • Spinal misalignment

  • Joint degeneration

  • Ligament sprains

  • Bone fractures

Treatment Approaches:

  • Functional physical medicine adjustments to improve joint alignment

  • Spinal decompression therapy for disc-related pain

  • PEMF therapy to enhance bone healing

  • Nutritional support to reduce inflammation and support joint health

Muscle and Tendon Pain (Myofascial Pain)

Myofascial pain stems from muscles, tendons, and their surrounding connective tissue (fascia). It is often described as a deep, aching, or burning pain that can be associated with muscle knots or trigger points. Unlike scleratogenous pain, myofascial pain tends to be more localized and may worsen with movement or specific activities.

Common Causes:

  • Muscle overuse or strain

  • Tendonitis

  • Fibromyalgia

  • Postural imbalances

  • Soft tissue injuries

Treatment Approaches:

  • Radial Pressure Wave (RPW) and Focused Shockwave therapy to break down muscle adhesions

  • Myofascial release and soft tissue manipulation

  • Targeted rehabilitation exercises to strengthen weak muscles

  • Proper hydration and nutrition to support muscle recovery

Nerve Pain (Neuropathic Pain)

Neuropathic pain occurs due to nerve irritation, compression, or damage. This type of pain is often described as sharp, burning, tingling, or shooting pain that follows the path of the affected nerve. It can be constant or intermittent and is commonly seen in conditions such as peripheral neuropathy, sciatica, and carpal tunnel syndrome.

Common Causes:

  • Diabetic neuropathy

  • Sciatica

  • Nerve entrapments (e.g., carpal tunnel syndrome)

  • Spinal disc herniation compressing nerves

  • Post-surgical nerve irritation

Treatment Approaches:

  • Neuropathy treatment programs with PEMF and K-Laser therapy

  • Spinal decompression therapy to relieve nerve compression

  • Functional medicine interventions to address underlying metabolic causes

  • Nutritional and supplement support for nerve regeneration

Internal Organ Pain (Viscerosomatic Pain)

Viscerosomatic pain originates from internal organs but can manifest as musculoskeletal pain due to nerve connections between organs and the spine. This type of pain is often dull, deep, and referred to other areas of the body. For example, gallbladder issues can cause right shoulder pain, while heart conditions can manifest as left arm pain.

Common Causes:

  • Digestive disorders (e.g., IBS, ulcers, gallbladder dysfunction)

  • Cardiovascular conditions (e.g., angina, heart attack)

  • Kidney stones

  • Gynecological issues (e.g., endometriosis, ovarian cysts)

Treatment Approaches:

  • Functional medicine evaluations to address digestive and metabolic imbalances

  • Chiropractic adjustments to relieve nervous system stress

  • Nutritional protocols for gut and organ health

  • Lifestyle modifications to reduce systemic inflammation
